Childrens Community Nursing Portsmouth

General Information

The Community Children's Nursing (CCN) Team provide specialist nursing care,support and co-ordination of care to children and young people with a range of complex health care needs and disabilities. These include those wuth life shortening conditions, chronic illnesses, palliative care needs, short term acute care needs and end of life care.

The CCN team work in partnership with acute and community paediatricians, therapists, G.P's, health visitors, school nurses, schools, children's services and voluntary organisations to help children and young people achieve their full potential and live as full a life as possible. This care is delivered in the community location that is the most appropriate for that child or young person e.g. home, school, respite centre.

The CCN team also provides nursing support into Mary Rose Special School and has a nurse therapist who provides loss and bereavement support and a dedicated citizens advice bureau worker who can offer benefit and other financial advice.   

The CCN team consists of a Community Children's Nurse Specialist Practitioner Team Leader with a team of 6 children's nurses, a nurse therapist and a citizens advice bureau worker.

No waiting times. All children meeting the referral criteria will be contacted within 24 hours of referral to arrange an initial visit and assessment.

Who do we work with?

All Children 0-19 years with a health need requiring nursing input who are registered with a G.P. in Portsmouth, East Hants or Fareham and Gosport localities.
