Childrens Community Nursing Southampton and West Hampshire

General Information

The Community Children's Nursing (CCN) Team provide specialist nursing care,support and co-ordination of care to children and young people with a range of complex health care needs and disabilities. These include those wuth life shortening conditions, chronic illnesses, palliative care needs, short term acute care needs and end of life care.

The CCN team work in partnership with acute and community paediatricians, therapists, G.P's, health visitors, school nurses, schools, children's services and voluntary organisations to help children and young people achieve their full potential and live as full a life as possible. This care is delivered in the community location that is the most appropriate for that child or young person e.g. home, school, respite centre.

The CCN team also provides nursing support into Cedars and Rosewood Special Schools and has a nurse therapist and support worker who provides loss and bereavement and supportive care.

The CCN team consists of a Community Children's Nurse Specialist Practitioner Team Leader with a team of 6 children's nurses, a nurse therapist and a support worker.

Who do we work with?

All children from 0-18 years of age with an identified nursing need, registered with a GP in Southampton, the New Forest (excluding Ringwood and Fordingbridge), Eastleigh Southern Parishes (excluding Eastleigh North) and Test valley South including Romsey.

Young people in specialist educational placements supported by an Education Health Care Plan will also receive a service up to transition into adult services at age of 19 years.

How do I get referred?

All enquiries by phone on 02380 538755 or email  and you will then be provided with a referral form to complete and return.  There are no waiting times. The children/families referred to the service will be contacted within 24 hours to arrange an initial visit.
