Child Outreach Assessment Support Team (COAST) Southampton

General Information

The service is made up of experienced children’s nurses who provide nursing support and assessment to your child during a period of illness.

You have been referred to us by a doctor to monitor your child’s progress.

We will speak with you over the telephone about your child and our decision to visit will depend on the concerns of the referring doctor, our team or your own concerns about your child’s illness.  If we need to visit your child we will always try to arrange a time that suits you and your family.

How will I know my child has been discharged from the service?

We will phone you or visit you to confirm your child is well enough to be discharged.  At this point you may discuss with the nurse any further concerns about your child’s recovery.

We will also inform your GP and/or health visitor to let them know that your child has been discharged from the service.

If your child becomes unwell outside of our working hours please contact the GP Out of Hours Service or visit your local Emergency Department at the hospital. In cases of emergency dial 999

Who do we work with?

We will work with you to arrange a home visit and we will always try to accommodate times that are convenient to you and your family.

We will monitor and assess your child’s progress and record any findings. We will discuss how you feel your child

is recovering, and together we will arrange a further plan of care for your child.

You will have access to the team for a period of approximately 48-72 hours.

If you or your nurse feel that your child’s condition is not improving the nurses will discuss this with the GP or, if necessary, the hospital team to agree the best way to manage your child’s care.

How do I get referred?

Your child will be referred to our Team by your GP or another doctor who has seen your child.

Our job is to monitor their progress and our aim is to provide high quality nursing care, ensuring a safe and speedy recovery at home.
